Hair loss can be frustrating for anyone but finding a suitable treatment for it is an arduous task, as well. In your quest, you may come through homemade remedies, some over the counter medications, and other stuff all claiming to treat a receding hairline. In reality, all these efforts go in vain once you realise that only a hair transplant can regrow hair on your bald scalp. But which type of hair transplant? The market is saturated with numerous acronyms; FUT, FUE, Sapphire, DHI and what not. Would you give a try to every treatment? Surely not. In fact, you will consider the most premium or the most advanced option among them and in that case, it is none other than DHI hair transplant.

Let’s dive into exactly how Direct Hair Implantation works, why it differs fundamentally from other methods, and whether it is the right move for your hairline.

What is DHI Hair Transplant?

DHI stands for Direct Hair Implantation. It is a highly advanced modification of the very popular and traditional FUE method. In DHI hair transplant, the way of taking the hair out of the donor area remains exactly the same as that of FUE. The difference lies in how the doctor put that hair graft back into the bald spots.

In a DHI procedure, the surgeon uses a specialized, patented medical tool called a Choi Implanter Pen. It literally looks like a thick metal pen with a tiny, hollow needle at the tip. Instead of manually pushing hair grafts into the skin with tweezers – as happens in FUE – the doctor loads the hair into this pen and injects it directly into the scalp.

What is the Difference Between DHI and FUE Hair Transplant?

The main difference between DHI and FUE hair transplant is the procedure of implanting the hair. In FUE, it is a two-step process. The surgeon takes a tiny steel blade and cuts numerous tiny slits all over the bald area. After that, the hair transplant specialist takes the extracted hairs and uses forceps to manually place them into the slits one by one. In a DHI hair transplant, the procedure is one-step. There is no need to make those tiny slits. The technician, however, loads extracted hair follicles into the hollow needle of the Choi pen. After that, they position the pen over your scalp, press the plunger, and the pen easily pierces the skin. The hair follicles, in this way, are placed in the desired area.

You experience less bleeding in DHI and consequently, the healing time is much faster than traditional FUE.

The Survival Rate of the Hair

Another difference between FUE and DHI is in terms of hair survival rate. Hair follicles are delicate and when they are removed from the head, they lose their blood supply. The longer they sit in a petri dish, the higher the chances they will die before they take  root in their new home. It is the same phenomenon that of a tree uprooted from a place only to be implanted again. The more it is kept in abeyance, the higher the chances that the tree will wilt.

Because DHI doesn’t require the surgeon to spend an hour making incisions into your head before implanting, the hairs spend significantly less time outside the body. Furthermore, because they are loaded into a protective pen rather than being squeezed and pushed with metal tweezers, the delicate root bulb doesn’t get crushed. This generally leads to a very high survival rate for the transplanted grafts.

Who Is the Ideal Candidate?

You are an ideal candidate for DHI if:

Your primary concern is filling in a receding hairline or adding density to a thinning crown. You have distinct bald patches but still have a good amount of hair surrounding them.

You have a healthy donor area.

You are a female and facing a pattern baldness.

However, if you are at a Norwood Stage 5 or 6 (the top of your head is completely smooth and you need maximum graft extraction to cover a massive area), you might be better suited to Sapphire FUE. Sapphire allows for a higher volume of grafts to be moved in a single, shorter session.

What Happens During a DHI Hair Transplant?

DHI is a meticulous procedure and therefore, the surgeon implants 2500 to 3000 grafts per day, taking around 6-7 hours. It is a pragmatic approach for both the patient and the surgeon as well. Extending it more than this figure can lead to exhaustion for both of them.

If you decide to go through with it, here is exactly what your day at the clinic will look like.

1. The Prep Work and Numbing

First, you will sit down with your doctor to draw out the final hairline. Once you agree on the look, a technician will give you local anesthesia. This whole process can take anywhere between 30-45 minutes. You will feel a few sharp pinches at the back of your head and on your forehead. But after a few minutes, your entire scalp goes completely numb.

2. The Extraction Phase

In this process, you will lie face down and the surgeon will extract healthy hair follicles from the donor area (often the back side of your head) using a micro-motor tool. These follicles will be placed in a chilled, nutrient-rich solution to keep them alive. This process can take anywhere between 1-2 hours depending on the number of grafts being extracted.

3. The Loading Phase

Now the implantation process begins. Here, the medical technicians assist the surgeon in loading the extracted hairs into multiple Choi pens. The surgeon’s job is to flawlessly implant the extracted hair follicles into the thinning areas. This phase completes in 2-3 hours.

4. The Implantation Phase

You might wonder what you would do in the implantation phase. Well, you can watch a movie or fall asleep during this part. You will not feel any harsh pain or any discomfort as the scalp will be numbed for the rest of the treatment.

What Are the Benefits of DHI Hair Transplant?

The primary benefits of DHI hair transplant over other hair transplant is the survival ratio of the extracted hair. Furthermore, the benefits include:

Unmatched Density:

The surgeon can safely implant the Choi pen right next to the existing hairs as there are no pre-made cuts, so this allows them to pack the grafts very tightly together, creating a dense look.

No Shaving Required:

In FUE or FUT hair transplant, you generally have to shave your entire head. In DHI, no shaving is required. The surgeon can easily navigate between your existing long hairs. This makes DHI an absolute option for women as well as for men who just want to lower their hairline without buzzing their whole head.

Natural Angles:

Hair doesn’t grow straight up. It grows at specific angles depending on where it sits on your head. The Choi pen allows the doctor to match those angles perfectly, meaning you won’t end up with that unnatural, spiky “doll hair” look.

Faster Recovery:

DHI has a faster recovery period as there are no such incisions unlike FUE or FUT, so there is less swelling.

What are the Downsides of DHI?

The first downside, though not for many, of DHI hair transplant is it takes a lot of time. In FUE, a surgeon can graft as much as 4,500 or more grafts per day, but in DHI, the maximum number of grafts that can be implanted in a day is up to 3,000.

If you are completely bald and need massive coverage, DHI might require splitting the surgery across two days.

Higher Cost:

Choi pens are single-use and quite expensive. Similarly, the surgeon carrying out the transplant requires more advanced training than traditional hair transplant techniques, resulting in a higher cost.

Highly Dependent on the Surgeon: A Choi pen in the hands of an inexperienced doctor is useless. It requires immense skill to control the depth and angle manually for thousands of clicks.

What Happens During Recovery: What to Actually Expect?

Here is the realistic timeline of your recovery period.

Days 1 to 3: Your scalp will be red and covered in tiny scabs where the implants were placed. The back of your head will feel tight and sore. You have to sleep on your back with your head elevated to prevent swelling. Do not touch or scratch the grafts under any circumstances.

Weeks 2 to 4: In this timeframe, the newly transplanted hairs will suddenly fall out but don’t panic. This is called “shock loss” and it is a completely normal biological reaction to the trauma of being moved. The actual root is still safely secured under your skin; it is just shedding the hair shaft.

Months 3 to 6: The resting phase ends and the new hairs finally begin to sprout. They will look thin and slightly wiry at first.

Months 9 to 12: The hair thickens, matures, and settles into its final texture. This is when you finally see the true result of your investment.

How Much Does a DHI Hair Transplant Cost in Dubai?

The cost of DHI hair transplant in Dubai depends on exactly how many grafts you need. A minor hairline touch-up requiring 1,000 grafts will obviously cost considerably less than a 3,000-graft crown restoration.

Moreover, final cost varies from clinic to clinic as it is largely influenced by expertise of the practitioner and overall image of the clinic. Clinics with successful results often charge more than less credible clinics and they are right in doing so. Similarly, experienced practitioners who have an established name in the realm of hair transplants charge a higher fee compared to those who are new in this field with less credibility.

Frequently Asked Questions

Is the DHI procedure painful?

The surgery itself is virtually painless. The only discomfort you will feel is during the first five minutes when the local anesthesia is injected into your scalp. After that, you could literally sleep through the rest of the procedure.

Does it leave noticeable scars?

No. Because it uses a micro-punch for extraction and a needle for implantation, there are no scalpel cuts or stitches involved. You will have tiny, microscopic white dots at the back of your head, but they are completely invisible once your hair grows out even a few millimeters.

How long does the surgery take?

Because it requires extreme precision, DHI is a lengthy process. Depending on the number of grafts, you should expect to be at the clinic for 6 to 8 hours.

Can the transplanted hair fall out again later in life?

No, the results are permanent. The hairs taken from the donor area at the back of your head are genetically resistant to DHT, the hormone that causes male pattern baldness. When they are moved to the front, they keep that genetic resistance. They will grow for the rest of your life.
